It is fairly common that the identity of the father is unknown. The adoption documents will list the father as the "Unknown Biological Father". His parental righst will be terminated in the adoption process. Just indicate that the father is unknown in the questionnaire.
Most of the adoptions that we do are where the father's whereabouts are unknown. If the father has abandoned the child and his whereabouts are unknown, the adoption documents will be filed with the court and the father will be served by publication. We will help you through this process.
All states will require older children to sign a consent to the adoption. Each state is different on the age that they require the child to consent, ranging from 10 to 14. In most states, the child will have to sign if they are 12 years old or older.
If you don't know some of the answers, such as a drivers license number or social security number, just leave it blank. Our adoption department will contact you if the information is needed.
Your child will receive a new birth certificate when the adoption is final. The new birth certificate will show the stepparent listed on the birth certificate as the parent at the time of birth.
Your child's name can be changed during the adoption process. Your child's new name will be listed on the new birth certificate that you receive when the adoption is final.
